At Desert Ridge Transitional Care Center, we are guided by what is best for our patients and residents. Their physical, social and psychological well-being is our chief concern. We offer an array of programs and services to give patients and residents many opportunities to engage in meaningful daily activities that they love. Our team provides uplifting, social, interactive and individualized activities to meet the needs of all those we serve.

Social Services

Our social service team assists both patients / residents and their families during transitional periods. We also provide essential information, manage requests and concerns, and help in care planning and discharge planning for those going home or to a lower level of care. We can arrange professional services for:

  • Dementia assessment

  • Individual therapy

  • Assistance in arranging notary

  • Stretching exercises

  • Music and memory programs

Nutritional

You are sure to enjoy the dining experience at Desert Ridge TCC Transitional Care. We are proud of our nutritious and appetizing meals serviced in a beautiful dining atmosphere. We also offer a private dining room for those special occasions.

The menus for Desert Ridge TCC are planned by a Registered Dietitian to promote proper nutrition and overall appeal through food preparation and presentation. Special diets can be tailored and are readily available to meet your unique nutritional needs.

You will always receive a variety of menu choices and visitors are always welcome to join you. If you prefer to dine in your own room, we will be happy to provide you with in-room dining.

Activities

Recreational activities can bring joy to life through companionship and laughter. You will be asked about your interests and hobbies on admission and we will design a activity program for you during your stay. Monthly calendars keep patients and residents informed of upcoming events such as birthday parties, movies, musical programs, outings and other special events. Friends and families are always welcome to visit and attend our special events.
